Dear Families of St. Susanna Parish Faith Formation Programs,
Thank you for sharing your children with us every week. We are blessed to be apart of your child’s faith formation. We sincerely hope that you have found this year’s program to be beneficial to your family. Please give us your input by filling out the survey on back of this sheet. We urge you to prayerfully consider enrolling your child for 2010-2011.
Enclosed please find the registration materials for next year. We will have an Open Registration opportunity for you to submit all registration forms. Open Registration will take place in the Hospitality Room in the Church on Sunday, March 21st after all masses. Come and register and have your questions answered.
To avoid a late fee of $30, registration forms and program fees must be received by Saturday, May 15th. Registration forms received after May 15th will be assessed the late fee. Checks may be made payable to St. Susanna. Registration forms may be remitted during open registration on March 15th, during any CFP session or via the Parish Office address listed above in the letterhead. For planning purposes, we ask that you not wait until late summer to register your child. Arrangements can be made if there is difficulty in paying this fee.
Every new student must have a copy of his or her baptismal certificate on file at St. Susanna. If your child was baptized at another church, please bring a copy of the baptismal certificate to the open registration day March 15th or include this information when you send in your forms.
Families preparing children for First Reconciliation and First Communion will receive special books and instruction and attend a retreat for each sacrament. A preparation materials fee is listed on the registration form.
