We will try to have most permission slips for Faith Alive, CORE, PA's, and LIFE published at least 3 weeks before they are due.  Check the publish date to make sure you have the most recent form.

Event permission slips are short forms that need to be filled out to participate in that event.  The General Permission slip should be filled out at registration (it is included in the registration packet) and will be kept on file.  If you have not registered and would like to participate in any event with St. Susanna Youth Ministry, please download one from the above link and include it with the event permission slip.
