Summer Event Release Form
All kids entering 6th-12th grades are welcome to join us forBreakout Games and Pizza Party on Wednesday, July 10th!
- Drop off at 10:45am at Break Out Games (6066 West Chester Rd, 45069 )
- Pick up at noon at Break Out Games OR at 1:30pm at Mellow Mushroom (9238 Floer Dr, 45069)
- Cost is $25.00 a person for ONLY the breakout room; OR $35.00 a person for the breakout room AND pizza party.
