Youth Ministry Registration
Please fill out the waiver to be added to the Jr. High and High School Youth Ministry contact lists.
This waiver only needs to be completed once per student each year. If you have already submitted a waiver for this student for a Youth Ministry event during the 2026–27 program year (September 1, 2026–August 31, 2027), you do not need to complete another one.
