Important Volunteer Information
All first time adult volunteers, prior to your visit please fill out the pre-registration form found here. Under Group Name please put “St. Susanna”.
Return volunteers can just bring their M25 ID card with them.
If there are any youth coming without a parent, please print off, fill out with a parent’s signature and bring with them THIS WAIVER FORM.
