Important Volunteer Information
All first time adult volunteers, prior to your visit please fill out the pre-registration form found here. Under Group Name please put “St. Susanna Church.”
Returning volunteers can just bring their M25 ID card with them.
Children under the age of 16 must be accompanied by a parent(s). Please see the chart below for the adult to child ratio requirements.
ALL 16 & 17 year olds coming without a parent, please print off, fill out with a parent’s signature and bring with them THIS WAIVER FORM.
